Second on Mates - none better for BC boots. I would choose him over Martin for BC boots. He has made 3 pairs of incredibly painful boots comfy for me.

Try a search...this topic has been brought many times and there are better threads on it.

I would trust any of the people mentioned in this thread to do the things you mention. 

Jim Mates is the guy I go to for "special" treatment, like making a custom stretching fitting out of wood to match the contour of my repaired achilles tendon, or analysing your stance/mobility in your boots. He's also a tele skier and perhaps more experienced than most at stretching problem areas around tele boot bellows.

I got my Garmonts at 2nd Ascent in spring 2005, skied them for the duration of the season and went back and had them fitted. Worked great. I may just have been lucky with "staff" fitter.
