Custom boot fitting recommendations?

I have some foot issues (super wide forefoot anda Morton's neuroma) and am looking for a super custom boot fit, i.e. may involve a bit of punching out the shell in addition to a good liner thermofit. Any recommendations in the greater Seattle-to-Bellingham area?

One update to the posts above is Martin is now at Crystal Mountain vs. sturtevants

for what its worth, the folks running Sturtevants boot-fitting department since Martin's departure are great. They've punched my AT boots before and cooked some liners for me and I've been very happy with their work.

Jim Mates over at seattle custom boot is amazing. I have super wide feet (4E), and I've taken several pairs of touring boots to him to have shells widened.

Martin at Crystal is from another world in fitting boots, you want them right, go see him.
