I bought a pair of Dynafit boots with the thermofit liners at The Mountain Shop several years ago and have been pleased. They didn't  have to do anything special to the boots (i.e. forming or grinding the shell) but they were very competent in helping me select the right boot and doing the thermoforming process.

Jeremy at the US Outdoor Store has done a good job doing boot work for me.

I just got a pair of Scarpas fit at the Mountain Shop and they did a great job. I bought the boot one size too small out of the box (foot would go to sleep in 15 minutes). I thought they would have to blow out/grind the sides for my foot but the liners fit perfectly after heating & molding. My first day touring on them and no blisters/pain at all. I think it was $40 for the fitting.
